How does monster level scaling work?

I know this is done through balance.txt ... Not exactly sure what values control this. But how does monster auto-leveling work? I know monsters level up with you, but what determines there min and max level for the region?

I would also like to know this, I find it annoying at this stage that the monsters are 3 to 4 levels below my character, on silver and with a SB of 48%. The only way to get "worthy" opponents is that I have to move on to the next region, but I still want to complete missions and explore a bit... any help any one.
