Quest Log Glitches

I have had several characters so far end up with red Xs on the left side of the logbook while the right side shows the quest was completed successfully. The quests Bandits and Plentiful Harvest seem to always show up with a red X and a couple of other quests sometimes show up as red Xs.

 

Also does it bug any one else besides me that the class specific side quests, the ones with blue question marks, do not show up in the logbook? The main class quests are in there only.
